WHAT IS IT? THE
NON SURGICAL FACELIFT EXPLAINED
At RIVKIN Aesthetics, one of our goals has always been to provide qualifying patients with safe, non-invasive alternatives to traditional surgical procedures. The surgical facelift or rhytidectomy, is one of the most popular cosmetic procedures being performed today. According to the most recent statistics from the American Society for Aesthetic Plastic Surgery, facelifts were the seventh most popular aesthetic surgery performed after liposuction, breast augmentation, abdominoplasty (tummy tuck), blepharoplasty (eyelid surgery), rhinoplasty (nose job) and breast lift.
Despite their relative popularity, facelifts are not being performed as much they once were, due in part to the explosion of non-surgical techniques such as our Non-Surgical Facelift. Most often, a facelift involves the removal of excess facial skin, tightening of the underlying tissues and remodeling of the skin on the patient’s face and neck. Facelift surgery is likely to be combined with a brow lift and eyelid surgery (blepharoplasty), and performed under general anesthesia. The surgery requires a couple of weeks of downtime. For patients whose goals would be better achieved through surgery, our board certified facial plastic surgeon is an internationally renowned facelift expert.
In his clinic, Dr. Rivkin has created a non-invasive alternative to facelift surgery, which is safe, relatively pain-free, and requires only a short recovery time. Dr. Rivkin’s liquid facelifta technique involves a combination of precisely placed Botox injections and injectable fillers to restore youthful contours and fullness, reduce lines and folds and make facial skin more smooth. Unlike a traditional facelift, eyelift, or brow lift, the Non-Surgical Facelift is practically painless.
There is no need for anesthesia. Dr. Rivkin uses tiny needles, most of the fillers contain lidocaine in the syringe and we use distraction techniques to take your mind away from the procedure. Combined with Dr. Rivkin’s famously gentle touch, we rarely even need numbing cream. You are entirely awake and alert during the injections, allowing you to see the results at intervals during the procedure and give feedback. You control the outcome.

FACELIFT PROCEDURE
Dr. Rivkin’s Non-Surgical Facelift procedure is an individualized tailored combination of treatments designed to refresh, rejuvenate and bring out the very best in your features. We combine precise and strategic Botox and filler injections with skin tightening procedures like Profound and skin smoothing technology like Fraxel laser to address aging in all of its manifestations.
When possible, Dr. Rivkin likes to have patients undergo Profound and Fraxel treatments prior to getting Botox and fillers. The skin tightening and smoothing effects of these two technologies take a few months to manifest. As this process takes place, the Botox and filler components of the procedure give patients immediate improvement they can see. By the time of the second Botox injection session, skin changes from the energy devices are visible and the synergistic effects of the procedure are well underway.
Botox Cosmetic injections smooth out lines and wrinkles caused by muscle contractions. Botox is used to relax the muscles of the forehead, between the brows and the crow’s feet lines around the eyes. It can also be used to decrease the appearance of vertical lip lines, to raise the corners of the mouth and to smooth out a dimpled or “orange peel” chin. Botox works well to lift the eyebrows – a procedure called Non-Surgical Brow Lift – making our patients look less tired. We also use it to decrease the size of the masseter muscles, softening the shape of the face – bringing it from a square, angular shape to a youthful soft oval.
Dr. Rivkin uses a variety of different fillers for his Non-Surgical Facelift procedure. His in depth understanding of the strengths and weaknesses of each particular filler permit him to tailor his selection to the individual needs of each patient.
He uses Voluma and Radiesse to replace lost volume in the cheek and cheekbone area, restoring vital structure lost with time. As we age, we feel that our face “drops.” We lose fat, muscle and bone everywhere, but most noticeably, in the cheek area. When this happens, our face loses support. Lines and folds form and the focus of the face drifts down away from the eyes. Re-volumizing the cheeks and cheekbones has the effect of re-establishing the structure of the face and lifting the focal point of the face back up to the eyes. Augmenting the cheeks also changes the shape of the face to be more of a youthful oval.
Age related volume loss of the face is not only limited to the cheeks, unfortunately. The two other major areas of volume loss that make people appear older and more tired are the temples and around the mouth. Hollowing of the temples makes the face look gaunt and older than necessary.
Dr. Rivkin uses Voluma or Sculptra to refill the temples during his liquid facelifts, softening the contour of the face and bringing back youthful fullness. Filling the hollows around the mouth corrects the impression of fatigue or sadness. Dr. Rivkin usually uses Voluma or Radiesse for this area. Radiesse or Voluma can also be used to fill in the space between the “jowl” and the chin, re-establishing a smooth jawline and camouflaging the jowl.
The next component of a Non-Surgical Facelift procedure involves filling the sunken tear trough region. Age related fat and soft tissue shrinkage under the eyes causes the appearance of a semicircular indentation. This is what we refer to when we talk about dark circles under the eyes. The condition is also called tear trough deformity. Shadow falls into this indentation, creating the appearance of dark circles. Dr. Rivkin uses Restylane, Versa or Voluma to fill in the tear trough deformity, eliminating dark circles.
The last major component of the Non-Surgical Facelift involves filling folds and wrinkles such as the nasolabial (smile lines) and marionette (frown lines) lines. Interestingly, these lines are usually already much improved after filling the cheeks and cheekbones. Re-establishing the cheek structure lifts the lower face and makes these lines less noticeable. Dr. Rivkin fills the leftover lines with Juvederm, Radiesse or Voluma, depending on their depth and appearance.

PROS AND CONS OF THE
NON SURGICAL FACELIFT
The injection aspect of the Non-Surgical Facelift procedure is virtually painless and usually takes less than an hour to perform. Recovery time is minimal. Patients may experience minor bruising, mild swelling, redness and tenderness, all of which should resolve within a week at most. The results of the Botox injections lasts about three to four months and the injectable fillers last six months to two years depending on which type of filler is used. We use mostly reversible fillers, which are easily dissolved or adjusted at any time if the outcome of the procedure is unsatisfactory for any reason. This procedure is not for everyone and should not be seen as a replacement for a surgical facelift. Our board certified facial plastic surgeon is always available to consult with patients who are not good candidates for Liquid Facelift. Most commonly these are people who have too much skin laxity for injections to be effective.
