WHAT CAUSES DARK
CIRCLES UNDER THE EYES?
Dark circles under the eyes can occur for several different reasons. They may happen as a result of a hollowness forming underneath the lower eyelid. This hollow groove (called the naso-jugal groove) forms as a natural result of tissue loss during the process of aging.
We know that as people age, they lose fat, muscle and bone all over the face. Under the eyes is one of the first areas to show these changes. Shadows fall into the groove, causing the appearance of under eye dark circles.
Bags under the eyes are caused by fatty tissue visibly protruding within the lower eyelid. Once again, a shadow falls into the groove underneath the bags and the person looks tired and older.
Another cause of dark circles under the eyes is from genetically inherited dark pigmentation of the under eye skin and blood vessels that can be seen through the skin.
The skin around the eyelids (otherwise known as periorbital skin) is the thinnest skin on the body (only around 0.5 mm thick compared with 2mm in other areas). Blood passes through the veins close to the surface of the skin and results in a bluish tint.
The thinner the skin, the darker the circles appear. Like many other skin conditions, dark circles are usually an inherited trait.

In the past the only remedy for bags under the eyes was an eyelid surgical procedure called blepharoplasty. This procedure involves removing or repositioning excess tissue through an eyelid incision. It carries all of the usual risks of surgery including bruising, swelling, scarring, eye damage, and anesthesia complications.
When performed by an experienced and capable plastic, facial plastic or oculoplastic surgeon, the results of surgical blepharoplasty are usually excellent. However, if results are not satisfactory, the procedure cannot be reversed. The other problem is, as we age, we lose the fat underneath our skin. Too often, ten years or so down the line, traditional blepharoplasty results in a sunken or hollow look, or eyelid skin that is too tight.
Topical creams and similar products are another alternative treatment. They can be of assistance temporarily for the mildest cases, but are unable to solve the problem otherwise. Most women tend to utilize heavy concealers and / or foundation to camouflage the issue.

Hyaluronic acid filler typically lasts 6-12 months when used on the face, but it can last longer in the tear trough area. If you're unhappy with the results of your tear trough injection, there is an option to dissolve the filler using a safe and effective enzyme called hyaluronidase. It won't harm your normal tissue. It's best to visit your injector for the dissolving procedure since they have knowledge of the specific areas that were injected.

You will see the final results after a full two weeks following the injections. It is common to experience some degree of swelling and occasional bruising, which can contribute to the appearance of lumps, bumps, or uneven distribution. During this period, you can gently massage the area with clean hands, apply ice, and consider taking Arnica to aid in healing.

When treating the under-eye area, injectors need to be cautious about the amount of product they use. The under eyes can retain fluid, leading to swelling caused by various factors like hormonal changes, salty foods, allergies, or colds. Fillers made of hyaluronic acid are hydrophilic, which means they absorb excess fluid from tissues. If too much water is absorbed, it can result in an overfilled look. Therefore, it's important to underfill the under-eye area to maintain a natural and smooth transition from the tear trough to the cheek. If you notice an overfilled appearance, don't worry! It's best to return to your injector, who can use an enzyme called hyaluronidase to dissolve some of the product.

Vision loss is an extremely rare but serious side effect that can occur immediately during filler injections. In our practice, we have never witnessed vision loss in the 20 years we have been performing filler injections. We often compare the risk of vision loss to that of a plane crash. Although statistically, plane crashes can happen, we still choose to fly because the risks are very small and we trust the expertise of the trained pilot in the cockpit. Similarly, with filler injections, it's crucial to have an experienced and cautious practitioner, whether they are a doctor, a physician assistant, or a nurse, to minimize the risks involved.

When people talk about ""under eye bags"", they usually mean two things: either deep tear troughs due to volume loss or bulging caused by fat pad herniation. Both of these issues can be addressed through fillers or surgery, and they typically don't see significant improvement with resurfacing lasers. Fillers are injected to restore lost volume or conceal the uneven appearance caused by the fat pad, creating a smoother transition from the under-eye area to the cheek. These filler results are temporary, lasting anywhere from 8 months to over 2 years.
