WHAT IS NON-SURGICAL
BLEPHAROPLASTY?
One of the first places where visible signs of aging begin to emerge is around your eyes. The delicate skin around the eye area is thin and becomes even thinner as you age. In addition, the skin in this area contains fewer oil and sweat glands, which makes it even more susceptible to the effects of aging. The most common age-related changes and complaints include dark circles, eye bags or swelling under the eyes. Dark circles are caused by fat loss over time in the area just under the lower eyelid. The hollow that forms looks like a dark circle because a shadow drops into the hollow. Eye bags are a weakening of the eyelid structure over time and protrusion of part of the the fat pad that surrounds the eye to where it looks like a small bulge. While these changes are not necessarily an indication of extreme exhaustion or serious illness, dark circles and bags certainly make you look more tired and older than you really are.
In the past, surgery was the only effective solution for puffy eyes and dark circles. Surgery to reposition or remove the fat under the eyes works well in patients whose main problem consists of significant protrusion of fatty tissue into the lower eyelid. Repositioning that fat smooths the under eye area and eliminates the bulge. For severe under eye volume loss, fat transfer can be a good option. Dr. Rivkin has found that the majority of people suffering from dark circles have a loss (not overabundance) of fatty tissue under the eyes over time. The appearance of puffy eyes is due to small amounts of fat herniating into the area just behind the lower eyelid skin. This progressive hollowing and fat herniation in the under eye area makes people look tired and older than they are.
Dr. Rivkin has created a non-invasive, safe, and cost effective alternative to surgery for these problems, the Non-Surgical Blepharoplasty procedure. Dr. Rivkin’s unique method for eliminating dark circles and camouflaging bags under the eyes is a painless, 15 minute injection procedure. He injects a select combination of hyaluronic acid fillers (Versa and Voluma) to even out the plane beneath the eye and raise the skin.
This fills the depression between the cheek and the eyelid and camouflages the “eye bags.” Small injections of the filler are placed in precise locations, filling in depressed areas, correcting the contours under your eyes. Filling the depressions eliminates the under eye shadows that are responsible for the appearance of dark circles and eliminates the look of protruding lower lids.
CANDIDATES FOR NON-
SURGICAL BLEPHAROPLASTY
The patients who benefit the most from non-surgical correction of dark circles are those who have primarily volume loss under the lower lid. If there is mild puffiness of the lower eyelid it can usually be camouflaged well with the filler injection. The depth of the indentation will determine what filler we use. More shallow tear trough hollows are easily corrected with Restylane or Versa, whereas deeper concavities need a more robust filler like Voluma to smoothly lift the tissues. During your consultation we will go over all of the filler options and determine what would be the best choice for you.
If a patient has prominent under eye puffiness, they would usually be better served with a surgical blepharoplasty procedure to gently reposition the fat and reduce the “eye bags.” In these cases, our board certified facial plastic surgeon will come in for a consultation and go through exactly what this will involve. He will explain risks, benefits and what to expect from the surgery. You will have the opportunity to go over before and after photos of the surgical and non-surgical options so you know exactly what to expect from either procedure.
PROS AND CONS OF THE
NON-SURGICAL EYELID LIFT
This procedure is virtually painless. Contrary to what you might expect, the under eye area is not very sensitive. Also, all of the fillers we use contain lidocaine, which numbs the area. For those with a lower pain threshold, we can also apply topical numbing cream. After the injections, it is possible to experience minor bruising, mild swelling or redness, all of which should resolve within a few days. During the procedure, we ice the area prior to injection to decrease the risk of bruising.
In order to further minimize the possibility of bruising, we recommend avoiding blood thinners for a week prior to your appointment. Blood thinners include alcohol, aspirin, ibuprofen, Advil, Aleve, vitamin E supplements, fish and flax seed oil supplements, ginko biloba and St. John’s Wort. It’s also a good idea to take arnica and bromelain tablets prior to and post procedure.
Results using hyaluronic acid fillers under the eyes are very smooth and last two to six years. This temporary method allows you to make adjustments as your face changes over time so that you will always have a natural look. An additional benefit of hyaluronic acid injectable fillers is that they are completely reversible with a simple enzyme injection if results are less than desirable or if an adjustment is needed.
Dr. Rivkin expects nothing less than flawless results from all of our signature non-surgical procedures. To that end, he recommends a touch up visit three weeks after your injection to check the results and adjust the filler, if necessary. During this visit, he may add to certain areas or even take some areas down slightly with the use of enzyme.

When people talk about ""under eye bags"", they usually mean two things: either deep tear troughs due to volume loss or bulging caused by fat pad herniation. Both of these issues can be addressed through fillers or surgery, and they typically don't see significant improvement with resurfacing lasers. Fillers are injected to restore lost volume or conceal the uneven appearance caused by the fat pad, creating a smoother transition from the under-eye area to the cheek. These filler results are temporary, lasting anywhere from 8 months to over 2 years.
For a more permanent solution, surgery can be a viable option. To ensure the best outcome, it's essential to consult with an experienced facial aesthetics injector or facial plastic surgeon.
